An application for the issuing an aeronautical assessment of aviation obstacles approval, or an amendment thereof shall be made to the Director in the appropriate prescribed form and accompanied by—
| (a) | appropriate fee as prescribed in Part 187; and |
| (b) | manual of procedure referred to in regulation 178.02.1. |
