An application for the issuing of a Grade Two air traffic service instructor (training organisation) certificate shall be—
| (a) |
made to the Director in the appropriate prescribed form; and |
| (i) |
the appropriate certificate of competency as prescribed in Document SA-CATS 65, signed by a rating assessment examiner; |
| (ii) |
a certified true copy of the air traffic service licence held by the applicant; |
| (iii) |
the appropriate fee as prescribed in Part 187; and |
| (iv) |
original or certified proof of his or her age. |
