On termination of employment an Employee is entitled to a certificate of service stating:

35.1	the Employee's full name,

35.2	the name and address of the Employer,

35.3	a description of the Bargaining Council by which the Employer's business is covered,

35.4	the date of the commencement and the date of termination of employment,

35.5	the title of the job or a brief description of the work for which the Employee was employed at the date of termination,

35.6	the Employee's remuneration at date of termination and

35.7	if the Employee so requests, the reason for termination of employment.
