An application for registration as a diver, diving supervisor, life-support technician, systems' technician, instructor, ROV pilot, ROV supervisor, chamber operator, compressed air worker, airlock operator or hyperbaric operations supervisor and any application for the reissue of a certificate that has been lost, damaged or destroyed, shall be made in the form and manner approved by the chief inspector.
