An application for the issuing of a type acceptance certificate for a Class I product shall be—
| (a) | made in the prescribed form and manner; and |
| (b) | accompanied by— |
| (i) | the appropriate fee as prescribed in Part 187; and |
| (ii) | proof of compliance with the provisions of regulations 21.04.3 and 21.04.4. |
